You can save time at your first appointment by downloading and filling out our New Patient information documents. Be sure to remember to bring any dental insurance card you might have and a photo ID, like a drivers license.
Please be on time for your appointment (come early if you want to fill out paperwork at the office). We believe missed and broken appointments are not fair to our other patients waiting to get seen. Don’t lose your spot! Please CALL US if you know you can’t make your appointment (at least 24hrs ahead) so we can have another patient fill your appointed time. Our dentists and staff are here to help you so we need you here too!
